What Do You Need to Know?

We welcome you to Living Well Primary Care! We appreciate the confidence and trust each patient places in us, and we will strive to create a partnership with you in order to maintain the very best health you can achieve!

* Office visits are for non-emergency conditions. If you are experiencing symptoms such as chest pain, shortness of breath or otherwise feel you are experiencing a medical emergency, call 911 or go to the nearest emergency room.

In preparation for your first appointment please complete the following tasks before you arrive:

Prior to arrival at your first appointment, please register with our Patient Portal. (Have your insurance card handy.) Fill out the required forms, including the Medical Health History form and Privacy Policy. Be sure to e-sign the Privacy Policy. Copies will be available at the office if you require them.

Contact your insurance carrier and ask them to assign Dr. Rochelle L. Collins as your Primary Care Physician.  If you need a Provider ID Number please contact our office at: 860-243-3315.
In addition to contacting your insurance company, please bring the following with you to your appointment:

  • Your insurance card.
  • A valid, government-issued photo-id card.
  • Any prescription medications you are currently taking.
  • A copy of your medical records from your previous Primary Care Physician.
  • Understand your insurance policy and what it covers. For example, if you are coming in for a general physical or vaccination please verify that your policy covers these services.
  • Female patients: Please know the date of the first day of your last menstrual period.  This can help Dr. Collins determine appropriate tests and/or medication.
  • All co-payments are due at the time of your appointment.
